IMPORTANT NOTES for making a Physio Analyser Scan Appointment for clients and staff of a Physiotherapy Clinic .
Appointments for staff and clients can only be booked by the authorised referring Physiotherapy Clinic. We don't accept bookings from the client direct.

Select an Appointment Time - We will call at the appointment time on the mobile number entered and conduct the scan. They can listen using earbuds, headset or on speaker. The person must be available at that time and should allow about 20 minutes for the scan. Please let them know that the time is allocated for them and that refunds are not available if they don't pick up, and miss their appointment.

Under 'Name" enter the name of the person receiving the scan
Mobile Phone: Enter the persons mobile phone to call. They will receive a reminder text message 24 hours before the appointment.
Email address: Enter your clinics email address. The scan results for your client will be sent to your clinic's email address, typically within 24-48 hours.
If the results are for one of the staff - for privacy purposes, you can enter their personal email address to receive the results direct.  
The "Date of Birth" This is required for database ID matching purposes and for frequency rating purposes to compare the persons frequency data with a healthy 'data base' person the same age. All data is securely stored and will not be sold to a third party.
"Time zone" - Please ensure the time zone/city is correct for the person receiving the call for the scan. We only provide the service for mobiles within Australia.

In the 'Comments Box' please ensure you add -
- a) the name of your clinic,
- b) the referring physiotherapist 

Credit card payment - When you are making the booking, a credit card payment of $110 will be required from either the clinic or the client. The clinic may prefer to include the cost in the client's Treatment Plan.
